Fat transfer is the right conversation if…

…your face looks tired rather than loose; the temples and upper cheeks have hollowed; under-eye shadows come from lost volume rather than skin; or fillers have stopped giving you what they used to. If skin is genuinely sagging, a lift addresses that and fat transfer refines it, and often the honest answer is both.

You need a donor area. Very lean patients sometimes do not have enough fat to harvest, and in that case we will say so rather than promise a result we cannot deliver.

How it works

Harvest, purify, place in small amounts

Gentle harvesting

Fat is taken with fine cannulas, usually from the abdomen or inner thigh, in a way that keeps the fat cells alive rather than simply removing them.

Purification

The fat is processed so that only viable cells are injected. This step is most of the difference between fat that survives and fat that disappears.

Micro-placement

Placed in many tiny amounts across different depths so it takes a blood supply. Overfilling one area is the classic mistake and it is why some faces look puffy rather than rested.

Nano-fat for skin

Very finely processed fat used superficially to improve skin texture, often around the eyes, where volume alone is not the whole problem.

Expect roughly 50 to 70 percent of the transferred fat to survive long term, and expect your surgeon to slightly overfill to allow for it. Anyone quoting 100 percent survival is not being straight with you.

What the months after really look like

  • You will look overfilled at first. That is deliberate and it settles, but it means planning around 10 days before any event.
  • Bruising around the eyes and cheeks is common for the first week and covers easily once the swelling drops.
  • The donor area is treated like small-volume liposuction: mild soreness, compression for a couple of weeks.
  • Fat needs a blood supply to survive, so no smoking, no heat, no massage of the treated areas, and no sleeping face-down for several weeks.
  • The final volume settles between month three and month six, and what survives to six months is yours permanently.

Fat transfer, the honest answers

How is this better than fillers?
It is your own tissue, so there is nothing foreign in your face, it feels natural, and what survives is permanent. The trade is a real recovery and a less predictable amount than a syringe of filler.
How much fat survives?
Around half to two thirds long term, which is why surgeons overfill slightly. If a clinic promises full survival, ask them to put it in writing and then reconsider the clinic.
Do I need enough fat to donate?
Yes, though less than people expect. Very lean patients occasionally have too little, and we will tell you before you travel rather than after you arrive.
Can it replace a facelift?
No. Volume and sagging are different problems. Fat transfer refills, a lift repositions. Many patients over fifty need both, and combining them in one operation is common.
Will it move if I lose weight?
Transferred fat behaves like the fat it came from, so significant weight loss can reduce it. Stable weight keeps the result stable.
